Provider Portal- Reports

The Reports Page is used to print reports. The R&R Update Form can also be found on this page.


To view a report:

  1. From the Dashboard, click the Reports button. 
  2. Choose an option from the drop down menu.
  3. If applicable, make a selection in the provider field. If you have multiple child care sites, there may be other sites listed here.

  4. Enter a date range to filter report results based on dates.

  5. Click the Print Report button. The report will be displayed.

  6. Click the printer icon in the top right corner to print a copy of the form.

To print the R&R Update Form:

  1. From the menu choose the R&R Update Form Report option.

  2. Click Print Report. The report will be displayed.

  3. Click the printer icon in the top right corner to print a copy of the form.

  4. Typically, providers will print the report and indicate any changes to their information. Then, return the form to the R&R department at the agency.
