Provider Listing by Program Report

The Provider Listing by Program Report displays a list of providers along with the families and children enrolled. The listing is grouped by program. The report includes the provider ID, provider name, child ID, child name, family ID, family name, schedule start date and schedule stop date.

Note: This report only list providers with children who have active schedules during the date range specified on the criteria form.

To View the Provider Listing by Program Report:

1. From the Reports Menu, select the Provider Listing by Program Report. The report criteria form will be displayed.
   
   
   
   
2. Enter a Start Date and Stop Date to filter for providers and children with active schedules during the designated date range.
   
   
3. Select a division or select "All" to include all.
   
   
4. Select a program group or select "All" to include all.
   
   
5. Select a program or select "All" to include all.
   
   
6. Click the Display Report button. You will have the option to print or save a copy to your computer.
