Family Form Reports: Enrollment Report
The Enrollment Report can be used to print an Agreement for Non-Subsidized Enrollment Form. Some agencies with center based programs use this as a contract with their non-subsidized/full fee parents.
This feature will only be used by agencies with Center Based Programs (CSPP, CCTR, Full Fee, etc.). When this functionality is not needed, CARE Administrators can hide the page by setting View rights to "No" for all users.
To view the Enrollment Report:
- From the Family Reports Menu, select the Enrollment Report. The report criteria form will be displayed.
- Enter the Effective Date.
- Click the Display Report button.
- Once the report is displayed, you will have the option to print or save a copy to your computer.
- The report header will be system populated with the agency name linked to the primary specialist for the family.
- The Latchkey, Preschool and School Age check boxes will be system populated based on the child's program. The following program group codes will need to be setup: LTKY (Latchkey), PRK (Pre-School), and SCHA (School Age). If any child linked to the family is enrolled in the associated program as of the report effective date, the appropriate box will be checked.
- A child section will be system populated for each unique child/schedule combination in effect as of the report effective date. The following fields will display for each child detail record: child name, provider name, child age, schedule start date, schedule stop date, schedule days (Sunday through Saturday), start time, stop time, rate type, PT rate and FT rate.
