Family Form: Documents Page


The Documents Page is used to store, view, and/or download scanned documents and/or pictures. Using this feature, you and other users at your agency can easily access documents, such as pay stubs, birth certificates, enrollment documentation, etc. It can also be used by child care centers to store pictures of the authorized people to pick up the children in the family.

This feature is especially helpful when an agency has multiple sites because the documentation is readily available from anywhere as long as the user has access to the CARE database. The filters at the top of the page allow users to search for documents by date, category, and/or description.


How to add a new document:

  1. Click the  on the [Add Family Document] line.

  2. Enter the date.

  3. Select the category. (Additional Categories can be added in the Document Category Setup Form)

  4. Enter a description (such as "Photo ID for Sally Johnson" or "Birth Certificate for Suzie Smith).

  5. Click the Choose File button.

  6. Find and select the desired file.

  7. When done, click the button to save the line.

  8. To add additional documents, repeat steps 1 - 7.

  9. To continue entering the family's information, go to the Family Form Menu, select each of the pages and enter the applicable information. A list of the pages can be found here.


How to download documents:

  1. Click on the checkbox next to the document(s) you would like to download.

  2. Click on the Download hyperlink. A pop-up message may appear if you are downloading multiple documents, click Allow.

  • To view the document, click on the file name.
