The Relationship Setup Form is used to maintain the list of options available on the /wiki/spaces/NSSD/pages/103612425.  This field is used to record the relationship between the child and his or her legal parent(s)/guardian(s).


 

 

The following entries are required for proper system functionality. Descriptions for each entry may be adjusted slightly, but changes to the codes may cause system-wide issues.

Optional: you can also enter "Mother" and "Father" as entries, both with the code "01".

Optional: you can also enter "Grandmother" and "Grandfather" as entries, both with the code "01".
