Provider Classification Setup Form

The Provider Classification Setup Form is used to maintain the provider classification options available on the Provider Form: General Page. Provider classification is most commonly used for tax reporting purposes.
The following entries are required for proper system functionality. Descriptions for each entry may be adjusted slightly, but changes to the codes may cause system-wide issues.
