CD-9400 Report

The CD-9400 Report is used to generate the CDE Enrollment & Attendance Register for Child Care & Development Programs. The CD-9400 Report is formatted according to State specifications. The report, as designed by CDE is formatted for printing on 11 x 17 inch paper. This report is generally used as a detailed breakdown for the numbers reported on the 9500 Report, however there are other reports that can be used for this purpose as well.



This report will only be used by agencies with Center Based Programs (CSPP, CCTR, Full Fee, etc.)When this functionality is not needed, CARE Administrators can hide the page by setting View rights to "No" for all users.



To View the CD-9400 Report:

  1. From the Reports Menu, select the CD-9400 Report. The report criteria form will be displayed.



  2. Select a division or select "All" to include all.

  3. Select the month and year for the report.

  4. Select an option for the Agency Owned Center filter. Under normal circumstances, "Yes" should be selected.

  5. Select a program group or select "All" to include all.
    Note: If you have multiple programs included in your 9400, you can create a "9400 Report Group" including all the programs that should be reported on the 9400. For more information on setting up program groups, click here.

  6. Select a program or select "All" to include all.

  7. Enter a Provider ID or click the Search button to use the Provider Search Form. Or, leave blank to include all providers.

  8. Select a classroom or select "All" to include all.

  9. Select a data source. Choices include "Attendance" or "Schedule."

  10. If necessary, click the Header button to enter report header information (Agency Name and Site Name). This information will be saved and only needs to be updated when there are changes.

  11. Click the Display Report button to view the report. You will have the option to print or save a copy to your computer.


Tip

  • Many agencies using NoHo CARE choose to update actual attendance times (in Attendance Entry) for variable schedule children only. For non-variable schedule children, agencies will enter absences only and keep the schedule time in and time out as is (based on enrollment) when submitting attendance. Then, they will choose the “Attendance” data source when running the 9400 report. For variable schedule children, it will use the actual times you entered in Attendance Entry. For non-variable schedule children, it will also use the times you entered in Attendance Entry (which are equivalent to the enrolled times- since you didn’t adjust for early departures and late arrivals).

  • If you want to limit the CD 9400 Report to a specific program, be sure to use the Program filter.



Group Designation Codes

These codes are displayed in the Group Designation column on the CD-9400 Report. The codes correspond to the grouping on the CDNFS-9500 Report.

Additional Information:

  • When a child has multiple special needs selected on the Child Form: Special Need Page, the report will use the special need with the highest adjustment factor.

  • If a program is selected for the Program filter on the report criteria form, the value entered in the Cost Center field on the Program Setup Form for the program will display in the Contract No. field in the report header. This field will be left blank if no program is selected on the report criteria form.

  • Days of non-operation (entered on the Provider Form: Non-Operational Days Page) are excluded from the Days of Enrollment and Days of Attendance counts on the report when the Reimbursable box is not checked on the Provider Form: Non-Operational Days Page.

  • FCCH status is determined by program group and age. In order to be in this category a child will need to be between 0-18 months at start of the reporting period and be in a program that is part of the “FCCH” Program Group and the child musts not have severely handicapped selected as a special need.

  • The tracking of certified vs. non-certified status is based on a program group code “PRVT” (Private/Full Fee). This program group code represents non-certified (private) status. Schedules assigned a program belonging to this program group will be considered non-certified. All other schedules will be considered certified.

  • Full fee (non-certified) children will only be included if they are enrolled at a center with at least one subsidized (certified) child. If all children at the center are full fee (no subsidized children), then the full fee children at that center will not be included. For example: There are five centers included in the report and four of them have a mix of subsidized (certified) and full fee (non-certified) children. The fifth center is only full fee children. The report will only count the full fee children in the four centers on the report.

  • If you don’t have a printer capable of printing on 11x17 paper, you can download a copy of the report and save it to your computer in PDF format.


Notes regarding data population:

  • The value for the Group Designation field will be the group designation code from the State based on need status, child age and care time.

  • The value for the Fee field will be left blank. CARE does not display a fee in this column because the fee cannot be displayed in a single field. The issue with subsidized families is that there is not enough room to display both the full time and part time fees. For full fee families, there is not enough room to show the fee and the fee type (monthly, weekly, daily, hourly, etc).

  • The value for the Amount Received field will be based on payment information entered in the Family Fee History Page for the service month and week.

  • The value for the Receipt No and Date field will be a concatenated string composed of the Reference No. (entered on the Cash Receipts and Adjustments Entry Page) and the Invoice Date from the Family Fee History Page
