Certificate Report


The Certificate Report allows users to print certificates for a single schedule or in bulk.



This feature will only be used by agencies with Provider Payment Programs (APP, CalWORKs, etc). When this functionality is not needed, CARE Administrators can hide the page by setting View rights to "No" for all users.



To create a new Certificate:


  1. From the Reports Menu, select the Certificate Report. The report criteria form will be displayed.



  2. Enter the Effective Date.

  3. Select a division or select "All" to include all.

  4. Enter a family ID or click the Search button to use the Family Search FormOr, leave blank to include all families.

  5. Enter a provider ID or click the Search button to use the Provider Search FormOr, leave blank to include all families.

  6. Enter a child ID or click the Search button to use the Child Search FormOr, leave blank to include all families.

  7. Select the address to use for the provider. Choose Main or Mailing.

  8. Select a sort option. Choices are "Provider" or "Specialist."

  9. Select the report format. Choices are "All Children" or "Each Child."
    Note: If you select t
    he "All Children" format, the certificate will include all children within the family with the same provider, schedule start date, and schedule stop date as the current schedule record.

  10. Check the Amendment box if this is a certificate amendment.

  11. Click the Display Report button. Once the report is displayed, you will have the option to print or save a copy to your computer.

Applicable Setup and System Options

  • CARE Administrators can choose the format to be used for the Certificate Report on the State Report Format Page of System Options.

  • There is a "NOA/Cert. Print Address" system option (on the State Report Format Tab of System Options). CARE Administrators can select "Main" or "Mailing" to specify which address should be used as the default.

  • There is a "Generate Certificate history from reports menu" system option (on the General Page of System Options). When this option is enabled and a certificate is created from this page, a copy of the certificate will be stored on the Schedule Form Reports: NOA/Certificate/Request for Documentation Page. When this option is not enabled, a copy of the certificate will not be stored on the Schedule Form.

  • There is a "Phone Display" option on the State Report Format Page of System Options. This option allows CARE Administrators to specify whether the specialist phone number or agency phone number should display on the Certificate Report.

  • There is a “Hide schedule stop date on certificate” option on the State Report Format Page of System Options. When this option is enabled, the schedule stop date will not be displayed on the Certificate Report.

  • There is a “Hide creation date on Default certificate” option on the State Report Format Page of System Options. When checked, the creation date will not be displayed on the Certificate Report.
    Note: this only applies if the "Default" format Certificate is selected.

  • There is an "Indicate on certificate when rates exceed RMR" system option on the State Report Format Page of System Options. When this option is enabled and the rates exceed the RMR, the memo section of the certificate will be system populated with text that reads “Rates are at or over the Market Rate Maximum.”

  • There is an "Indicate on certificate when there is a non-zero co-pay on the schedule” system option on the State Report Format Page of System Options. When this option is enabled and there is a co-pay entered, the memo section of the certificate will be system populated with text that reads “Rates are at or over the Market Rate Maximum.”

  • There is a “Print barcode on Certificate” system option on the State Report Format Page of System Options. When this option is enabled and the "Default" format certificate is being used, the certificate will include a barcode in the page footer. Note: When printing certificates from the reports menu, the barcode will only be included when the “Each Child” option is selected on the criteria form. The barcode consists of a 15 character concatenated string composed as follows: leading and trailing asterisks and the schedule ID padded to 13 characters with leading zeroes. 

  • The "Combined Schedule" format lists all schedule information for the unique provider/child combination (i.e., if the provider and child have more than one schedule, all of the information will be displayed on the "Combined Schedule" format Certificate).
